Check them out here! Form 10 - Initiated new members? Click Here to submit your Form 10. Dispensation Request - Need special permission for something? Use this form. Driving and Insurance Policies – Each Chapter is required to have on file (a copy must be in the possession of each advisory while traveling with DeMolays). Use this form. Health Information – Required to be on file for all DeMolays and must be carried by advisor when traveling. Use this form. Youth Protection – Nothing is more important than our youth and DI has developed a DVD that is required to be shown at least twice a year. It is in small discussion modules and should be shown at least every other month. The Youth Protection – Proof of Attendance is required to be filed with Michigan DeMolay twice a year. (Before each Installation) Use this form. Yearbooks and Fundraising - Michigan DeMolay produces an annual Yearbook that commemorates the past year. This book provides funds to the Chapters and Michigan DeMolay through the sale of the advertisements which appear in the yearbook.
